Processes for Vaccination

COVID Vaccine - District 4 Public Health

Process for all drive-thru PODS:

Due to enormous demand, there is a long wait time. Please be patient.

We will only take calls to schedule appointments when we have vaccine available.

You will receive a date, time, location, and confirmation number when you schedule your appointment; please take note of the confirmation number and bring it with you to your appointment.

Arrive at your scheduled location no more than 10 minutes early. Get in line (you will not leave your car). Follow the line until you get to the nurse. Bring your photo ID (work ID for healthcare, first responders, and LEOs).

Present the nurse with ID and signed consent forms. If you don’t have access to a printer, or if you forget your consent form, we will have paper copies on site.

Roll up your sleeve and get vaccinated!

The nurse will give you a card with the name of the vaccine you received as well as the date that your next dose is due. The date of your second appointment is written on the back of your vaccine card. You do not need to call to make your second appointment. You will return to the same location as the first dose, at the same time as the first dose. Continue to check our website as well as our social media pages for updates on availability.

Pull forward and sit in your car for 15 minutes. If you still feel fine, you are free to leave. If you feel bad or think you might be having an allergic reaction, honk the horn and someone will come check on you.

 

Process for on-site clinic PODS:

Process to make second appointment:

 

If you received your first dose through District 4 Public Health:

You will return for your second dose on the date on the back of the card that was given to you at your first appointment. You will return to the same location, and at the same time as the first dose. You may also visit our website to find your second appointment date here

What do you need to bring to your second appointment? Bring the card with the vaccine name and due date with you to the second appointment as well as your ID and your vaccine consent form. If you forget the card, you will have to pull out of line and a nurse will have to look you up in GRITS, causing a significant time delay.

Follow same guidance as your first dose (no more than 10 minutes early, present ID, consent form, and vaccine card…wait 15 minutes before you leave)

 

If you received your first dose through another public health district or another healthcare provider:

Call: (888) 457-0186 to schedule an appointment OR you can go schedule an appointment online here  to be placed in the best available time slot. Please keep in mind, this time slot cannot be more than 4 days BEFORE your 2nd dose is due, but it may be after the second dose is due.

 

Canceling and Rescheduling

To cancel your appointment, please give us 48 hours’ notice if at all possible using this link: www.district4health.org/about-us/contact/  If you aren’t able to give us that much notice, that is ok. It is very important that you let us know if you need to cancel so we can give that slot to someone else!

IT WILL BE VERY DIFFICULT TO RESCHEDULE YOUR SECOND DOSE, PLEASE MAKE EVERY EFFORT TO MAKE TIME FOR YOUR SCHEDULED APPOINTMENT. WE CANNOT GUARANTEE THAT YOU WILL BE ABLE TO GET A TIME SLOT FOR A DIFFERENT DATE OTHER THAN THE ONE GIVEN TO YOU.
