Online Appointment Scheduling

Schedule Your Health Department Appointment Online

We invite you to visit District 4 Public Health’s new online scheduling portal, where you can view your county health department’s available appointment openings and then book your appointment online!

If you don’t have access to a computer or someone who can help you sign up for an appointment online, contact our Call Center at (800) 847-4262; our representatives will be happy to schedule your appointment for you! The Call Center is available from 8 a.m. to 5 p.m. Monday through Friday.

Through the online portal, you’ll first choose your preferred health department and then view a list of services that can be scheduled either online or by phone. (Services listed in red will need to be made by phone because of the varying times spent with each patient.)

If the health service you need is available through online scheduling, you can then reserve an appointment time with our simple calendar tool. You’ll then complete a brief form that includes your name, date of birth, reason for visit, and cell phone number.

Once you make your appointment online, you’ll receive a text confirmation along with a text-message reminder prior to your visit. The text confirmation will include a link if you need to cancel your appointment. You also may call your health department directly to cancel.

Appointments for routine health services are generally available within a few days, and occasionally you may even be able to make same-day appointments.
