Family Nurse Practitioner Services

Spalding County Health Department has a board-certified Family Nurse Practitioner on staff, Jasmine Webb, FNP-C, who provides services for children and adults.

Appointments preferred: Same day appointments may be available. To make an appointment please call 1-800-847-4262. Please have your insurance information readily available when you call to make an appointment. Most insurance plans accepted

We strive to offer low-cost services to our customers. Many services are based on income. Payment is expected at the time of service. We accept cash, Visa, Mastercard, American Express, and debit cards. No one is denied services for inability to pay.

Did you know that you may qualify for WIC services if you are pregnant, just had a baby or are breast feeding your infant? Your child may qualify up to their 5th birthday. If you qualify you receive a nutrition assessment, health screening, nutrition education, referrals to other health and social services, vouchers for healthy foods and much more.  Here you can complete an online application to see if you qualify.
