Due to the public health state of emergency surrounding COVID-19, the State Office of Vital Records has suspended all walk-in services until further notice. Please inform everyone in your office and local partners of this change immediately. Also, please advise constituents to request birth and death records online using one of our approved third party vendors (ROVER, Go Certificates, or VitalChek). For changes or corrections to a vital record, please continue to mail requests, along with the required documentation, to our facility. Did you know that you may qualify for WIC services if you are pregnant, just had a baby or are breast feeding your infant? Your child may qualify up to their 5th birthday. If you qualify you receive a nutrition assessment, health screening, nutrition education, referrals to other health and social services, vouchers for healthy foods and much more.  Here you can complete an online application to see if you qualify.
