Dear Parents/Guardians:

This influenza (flu) season we would like to help protect our student body by providing flu vaccines during school hours. The Heard County Health Department will be offering injectable flu shots. The following vaccine will be available:

Inactivated Influenza Vaccine (IIV): an influenza vaccine that is given as a shot.

If you would like to have your child vaccinated during the above school based flu clinic please:

1. Review the Vaccine Information Sheet (VIS).
2. Complete, Sign, date, and return the student consent form your teacher before September 21.

Staff complete, sign, date and return the staff consent form to your school before September 21.
